All Participants need to fill out the three forms below and send/give to the Director

Alll IUP students need to fill out the 2 forms below in addition to the ones above.

2. IUP Risk and Release/ Statement of Commitment Form - these forms need to be notarized. Marian Stockdale at the HUB can notarize documents for free by appointment only (Marian.Stockdale@iup.edu and 724-357-1318)

Finally, the University requires you to purchase an International Student Identification Card (ISIC). The card currently costs $22 and processing time takes 3 to 6 business days. The card provides travel discounts, cultural discounts, and basic insurance coverage (medical emergency, evacuation, and repatriation). For more information visit www.isic.org. First visit http://www.iup.edu/registrar/clearinghouse/enrollment.htm and print out an enrollment verification letter. Then download the application and follow the instructions on page 1.
